Complimentary Ticket Form

(for nominees, volunteers, and board members)

Congratulations on your nomination!  We are hopeful that you will be able to attend this year’s awards so that our entire membership can celebrate…you!  This form describes the process for claiming your complimentary ticket(s) and also allows you to request additional paid tickets for your guests.

As a nominee, you will receive one complimentary ticket (as long as your membership is active).  If you are part of a band that is nominated, only those band members who are individual members of Midwest CMO will receive a complimentary ticket.  The exception to that is if the whole band is performing.  Any musician performing on stage will receive one complimentary ticket.  Don’t fill out this form too soon, as we plan to announce performers as soon as possible and it may affect your number of comp tickets.

Whether you plan to buy additional tickets (for a spouse/family, other band members, managers) or not, please fill out the form below.  For example, if you are nominated and want to attend with your spouse and kid, you will need to use the form to note for the La Crosse Center how many additional tickets you’d like to purchase together (2 additional in this case).  If you are coming on your own and only need your comp tickets, simply enter a zero for additional tickets.

This form will need to be submitted no later than noon on Friday, February 20, for the best seats.  After that time, you can still use the form to receive your complimentary ticket, but we will not be able to reserve you seating closet to the stage (you will just be in line with the public who will be purchasing tickets at that point).  The La Crosse Center box office will call you for payment for your additional tickets ($74 including taxes and venue fee, plus $10 processing per order) starting Mon, Feb 23.  They will seat in order of date/time of submission of this form.
