Congratulations on your nomination! We are hopeful that you are not only able to attend this year’s awards but possibly perform as well. This form will provide Midwest CMO your availability and potential performance options. Please complete this form as soon as possible, as we will be setting performances within days of the nomination announcements.

Please note: We desperately want to include all nominees in the awards show, but putting 61 artist nominees in a 3-hour show is simple not possible. As such, we cannot guarantee a performance slot for everyone. Collaborations of multiple nominees certainly help every year, and there is a place below for your ideas in this regard. We will prioritize those with multiple nominees and those from categories such as Entertainer, Male/Female Vocalist, Band, Album, Song, and Breakthrough. Again, we apologize that not all will get to perform.
